My student would like to participate in the Bradenton Christian Reformed Church Elevate Youth Group Junior High Retreat.

NOTE: The cost of TreeUmph is $40 a student. Everything else will be covered by the church. We encourage the submisison of payment using our online giving system, please select: "JR High Activity Deposit" when doing so!

I agree that I am authorized to and do give my permission for the student listed above to attend Retreat activities (TreeUmph Ropes Course, sleepover at the church, service activities around Bradenton/Sarasota) sponsored by BCRC. I give permission for the leaders of the organization to secure needed emergency treatment for the minor named above. In case of accident or injury during activities, I release the leaders from all liability not covered by insurance.
